Disease Management 

What is Disease Management? 

Disease Management is an important part of your care team at Malmstrom. The main goal is to provide you with information and support about your health condition, helping you make lasting changes to your habits. With personalized health coaching, you’ll better understand your condition, learn why certain habits may develop, and work on building skills to make helpful habits a consistent part of your life-no matter what challenges you face.   

What conditions qualify for Disease Management? 

You may qualify for Disease Management health coaching if you have: 

  • Weight concerns 
  • Elevated blood sugar (Prediabetes, newly diagnosed Type 2 Diabetes, or uncontrolled Type 2 Diabetes) 
  • Elevated blood pressure 
  • Elevated cholesterol 
  • Perimenopause symptoms 
  • Difficulty sleeping 
  • Newly diagnosed or uncontrolled asthma 

You can also meet with Disease Management if you have a family history of these conditions or want coaching to help prevent certain health issues from developing. 

This service is available to TRICARE Prime beneficiaries, including active duty service members, retirees, dependents, and activated Guard/Reserve members and their dependents who are at least 18 years old.

Lifestyle Coaching Program 

Looking for support to change your habits? The Lifestyle Coaching Program can help. 

Making small changes in your routine can lead to big improvements. This program helps you identify habits that may be holding you back and teaches you how to create new, healthier routines. By focusing on small, achievable steps, you can retrain your brain and body to enjoy new, helpful habits, making them stick in the long run.   

You will work one-on-one with Malmstrom’s Disease Management RN. The program uses two proven, evidence-based curriculums: 

  • CDC’s Diabetes Prevention Program “Prevent T2” 
  • VA’s National Center for Health Promotion and Disease Prevention Program “MOVE!” 

Each session will cover topics like Nutrition, Activity, and Emotions, and you’ll set personalized goals. Each session builds on the one before, helping you progress over time.   

You’ll meet every 2 to 4 weeks to check in on your goals, making sure you are on track and staying motivated.   

This program also pairs well with our group classes like Health Promotion’s 360 Wellness class and Mental Health’s Unified Protocol class for habits and emotions. If interested, In-Body Composition Analysis is available to track your progress. This program is also a good option for Active Duty Body Composition Improvement Program.
