What is EFMP-M?
A mandatory DOD program which ensures active duty service members are enrolled due to their dependents that have a special medical, behavioral, and/or educational need(s).
What conditions qualify for EFMP-M?
You may qualify for EFMP-M if you have (not exhaustive):
- Frequent follow ups from PCM more than once a year for a chronic condition including mental health conditions
- Referral for Specialty Care
- Behavioral Health Appointments (ADHD/ADD, Depression, Anxiety)
- Developmental Delays
- Autism
- Asthma
- Durable Medical Equipment need (eg: CPAP, feeding tubes, hearing aids. etc)
- Individual Education Plan or Individual Family Service Plan
EFMP-M Requirements for a Permanent Change Station
Your next assignments EFMP-M requirements depend on where your next assignment will be located. It is important once you receive your assignment notification to go to MyVector for a Travel Screening Questionnaire. A TSQ will populate if your RNLTD is within 8 months. If your TSQ is not loaded (& your RNLTD is within 8 months) within 8 business days, please contact EFMP-M.
